Our goal is to return each patient to his or her full function in an encouraging, kind, and positive environment.
We work with all types of patients – individuals of all ages, from all walks of life, and with completely unique needs. From young to old, and everyone in between, our therapists and trainers are here to help.
Many of our patients and clients are injured, have undergone surgery, or suffer from medical conditions. Others experience pain in their daily activities. We also assess and treat athletes participating in a wide range of sports and work with our local high school teams and coaches.
If you have a prescription for physical therapy from a physician, you may begin therapy within 48 hours of your call to us. If you are unsure that therapy is the best next step, you may come in for a free screen and we will assist you further. To schedule a free screen, simply click on the link to select a date and time for your screen. Dates and times are determined by location and availability.
The length of your assessment and treatment sessions depends on a number of things, mainly your individual needs. Usually, each session lasts approximately 60 minutes. Most of our patients and clients have sessions 2 or 3 days a week, although this can vary.
The length of your treatment is determined by the extent of your injuries, pain level, personal goals, and many other factors. Once you receive an evaluation, your physical therapist can work with you to determine a realistic timetable for treatment.
Although some of our patients have a physician’s prescription, this is not necessary for physical therapy. However, most insurance companies require a physician’s prescription to allow us to bill for our services. We can assist with questions regarding prescriptions and insurance coverage when you come in for your free screen. Please click on the link below to schedule your free screen.
We offer free screens to the public at all 4 of our locations. The free screen includes a 15-minute private consultation with one of our physical therapists to discuss your pain or functional limitation difficulty.
During the session, the therapist will listen to your concerns and observe your functional difficulty, measuring areas of deficit as they arise. After this screen, your therapist will share their findings with you and help you to determine the next steps. Your therapist will also generate a letter for your doctor that outlines your functional difficulty, the therapist’s findings, and the recommended next best action steps will be discussed with you. Also at this time, we can answer any insurance questions you may have regarding your therapy.
On your first visit, you’ll be asked to complete paperwork including patient information and medical history forms. Next, you’ll receive an evaluation by a physical therapist in one of our private treatment rooms. We ask that you share your concerns and ask questions during your visit. The physical therapist will work with you to determine a plan for your individual treatment. Most times treatment and home exercises will be provided this day as well.
All levels of discomfort are monitored by the 3D PT staff. If you experience pain during your therapy visit, please speak up, and let us know so we can adjust your program accordingly.
You will work with one of our trained staff members at each visit. This person will guide you through your treatment. You will see the professional in charge of your care for that day at each visit.
At 3D PT, we work as a team to achieve the best results for you. This means you may not always see the same team members each time you visit. However, we have a very open environment and approach to treatment, so, if you feel other team members need to be made aware of your treatment, please feel comfortable to touch base with them.
Please bring your driver license and patient forms, a list of medications you are taking, and your insurance card(s).
You can fill out these patient forms online:

During each visit, please communicate your progress to our team members. We will also measure your clinical progress at regular intervals and communicate this information with your doctor.
